Vell... You could try taunting. It sounds vauge, but it's easily done. You have Gerard, in his room, the guys give him food, and leave. They don't shut the door (part of the plan). Gerard, being crazed to get out of there, he runs (not bound or anything, just him and his denailed hands) to the door, but right as he gets to the door, it shuts. He goes back to the bed. They open the door again. Repeat. Might need some kind of drug involved in there... That, and, well... There's this one thing people do in one country in Africa... When a boy comes of age, he's taken from home to one building with other boys his age. For years they're there, but for one purpose: they take razors to their skins repeatedly. All over their bodies. And then they wash them, which hurts like a bitch, from what I gather. The torturees can do that to Gerard one day and take alcohol to the cuts... Jeeze, I think too much. They could also do ack-you-punk-sure (I sounded it out, cos I have no fuckin' idea how to spell it right). It's supposed to calm, but you're being stuck with hundreds of needles. Gerard is afraid of needles. Then you press down on them lightly, just to the point of a little blood.
